You will be completing 3 assessments at this site: HPI, HDS &, MVPI
* Using Microsoft Internet Explorer 4.0 (or higher) or Netscape Navigator 4.0 (or higher), type in the path name:

* Enter the User ID and Password you received from your coach, then click “go” and then “continue.”
* Fill in the participant information (this is you). Please enter the following information carefully and completely to ensure successful report processing. Please do not enter any commas, hyphens, apostrophes, or initials. All of this information is used to select the best normative comparison for your report and is confidential. Your social security # is used as a means of identifying and linking your reports. If you do not wish to release your social security #, you can enter all 0’s into this field. If you like, you can make up your own ID # instead of using the social security field, but you must remember it and use it for each report that you take!

* Click on “Submit.”
* Click on “Start Assessment” of the survey you wish to take. It is important to spend only the 20 – 30 minute timeframe versus spending a lot of time analyzing each survey question. There are no “right” answers to a test of this type. Your first answer is generally your best answer.
* Select answers by clicking on the appropriate circles.
* Click “next” to advance to the next page of questions.
* When you are finished, click on “Submit” button and either complete the next assessment or exit program.
* You must finish the entire assessment in a single session.
Note: if you click on “Stop Assessment” you will be returned to the participant menu and your responses will not be saved.
